It is always the physician’s responsibility to determine coverage and submit appropriate codes, modifiers, and charges for the services that were rendered. Physician must contact local carrier/payor for interpretation of appropriate coverage and coding policies. When using our equipment or procedure to control pain, it is also necessary to provide adequate S.O.A.P. note documentation in a patient file. It is also recommended this includes  documentation of the medical necessity for any and all treatments.
